Crusader Medical Care provide specialist nursing care to clients with complex care needs or profound disabilities through Slow Stream Rehabilitation, Complex Disability Management and Neuro-Palliative Rehabilitation. We are able to accommodate a diverse clientele group from the ages of 18 years and upwards.

Crusader Medical Care employ a holistic person-centred plan of care, using recognised models of nursing to identify the individual needs of each client and enhance their overall quality of life. Our highly skilled, multi-disciplinary team work with our clients and their families to create and continually review personalised care programmes based on the individual's changing needs. We provide 24 hour specialist care to our clients in accordance with our Philosophy of Care and Quality Management System. All clients are assisted in exercising choice and control over their personal lives. Emphasis is placed upon the importance of independence, choice and dignity for our clients. Each individual has the right to lead an independent and fulfilling life, through the lifestyle of their choosing.

 

Care Pathways

“Rehabilitation is a process of assessment, treatment and management by which the client is supported to achieve their maximum potential for physical, cognitive, social and psychological function, participation in society and quality of living”.

Client goals for rehabilitation vary according to the recovery trajectory and stage of their condition. Our specialised rehabilitation services are provided along three main care pathways:

Slow Stream Rehabilitation

For those recovering from a “sudden onset” or “intermittent” condition, where the client works towards specific outcome goals over a prolonged period with the aim of significant functional improvement. Our care staff work with clients to improve independence in daily living activities with the aim of returning to live in the community or transfer to a less specialist setting.

Complex Disability Management

For those with stable or progressive conditions where the goal is focussed on maintaining existing levels of functioning and participation; compensating for lost function (e.g. through provision of equipment / adaptations); or supporting adjustment to change in the context of deteriorating physical, cognitive, and psychosocial function. Our care staff work with clients to both prevent complications and enhance participation and autonomy despite the restrictions of severe activity limitation.

Neuro-Palliative Rehabilitation

Where the goals are focussed on symptom management and interventions to improve quality of life during the later stages of a progressive condition or very severe disability, this care pathway is found at the interface between rehabilitation and palliative care. Through the use of Advance Care Planning our care staff work towards the preservation of autonomy and relief of distress of clients.
